Joining a scheme – necessary formwork

• Parents must sign a Salary Sacrifice Agreement, a formal agreement between an employer and an employee. For more information, click here.

• Carers must complete a carer form and send Sodexho Pass a copy of their carer registration / approval certificate.

Leaving the scheme

As part of the Salary Sacrifice Agreement, parents agree to take the vouchers over an agreed fixed period (usually one year) and should remain on the scheme for that period of time. Parents may leave the scheme under exceptional circumstances e.g. birth, marriage, divorce. Any other reason would have to be agreed on an individual basis with the employer.
